DESCRIPTION

Florida Department of Health in Polk County (FDOH) provides dental services that include, but are not limited to, exams, cleanings, X-rays, fluoride treatments, sealants, pain relief, amalgam fillings, composite filings, sedative filings, and routine tooth extractions. Services are provided to Polk County residents whose income is at or below 200% of the Federal Poverty Level (FPL) guidelines and who are uninsured or underinsured.

 

FDOH has dental clinics in Auburndale, Bartow, Haines City and Lakeland.

 

This contract will be effective October 1, 2022 through September 30, 2025. A fee schedule has been established to reimburse FDOH an agreed upon rate per dental visit.

 

This funding request was presented to and approved by the Citizen’s HealthCare Oversight Committee (COC) at the March 18, 2022 meeting.

 

RECOMMENDATION

Request Board approve the contract with Florida Department of Health in Polk County to provide dental services to eligible low-income county residents.

 

FISCAL IMPACT

Funds are budgeted in the Indigent Health Care Funds.
