Approve the contract with Florida Department of Health in Polk County to provide primary care services to eligible low-income county residents who are not Polk HealthCare Plan members. (total amount not to exceed $728,000 annually)

DESCRIPTION
Florida Department of Health in Polk County (FDOH) provides primary care services in an effort to appropriately link residents to needed medical care to improve overall health and to decrease local emergency room overcrowding by individuals that can best be served in a primary care setting. Services are provided to Polk County residents whose income is at or below 200% of the Federal Poverty Level (FPL) guidelines and who are uninsured or not otherwise Polk HealthCare Plan members.
This contract will be effective October 1, 2022 through September 30, 2024. A fee schedule has been established to reimburse FDOH an agreed upon rate per patient visit.
This funding request was presented to and approved by the Citizen’s HealthCare Oversight Committee (COC) at the March 18, 2022 meeting.
RECOMMENDATION
Request Board approve the contract with Florida Department of Health in Polk County to provide primary care services to eligible low-income county residents who are not Polk HealthCare Plan members.
FISCAL IMPACT
Funds are budgeted in the Indigent Health Care Funds.
