DESCRIPTION
Fire Rescue requested Procurement to solicit proposals for the purpose of performing physical examinations for approximately 700 firefighters and department employees. The exams check for medical preparedness based on requirements of the National Fire Protection Association and in-depth consideration of essential functions. These essential functions are what members are expected to perform at emergency incidents.
Request for Proposal (RFP) 22-411, Firefighter Physicals was issued, and one vendor submitted a proposal. On August 16, 2022, the Board approved the selection committee’s recommendation to enter into negotiations with the proposer, Life Extension Clinic, Inc., dba Life Scan Wellness Centers. The attached agreement is the result of those negotiations. Life Scan Wellness Centers is also the incumbent provider.
The term of the agreement is for an initial three-year term, with two, one year term renewals.
RECOMMENDATION
Request Board approve agreement with Life Extension Clinic, Inc., dba Life Scan Wellness Centers of Tampa for RFP 22-411, Firefighter Physicals for an estimated recuring annual expense of $695,000.00.
FISCAL IMPACT
Funds are available within Fire Rescue’s budget.
