DESCRIPTION
Health and Human Services Indigent Health Care (IHC) program is proposing a collaboration with United Way of Central Florida, Inc. (UWCF) to increase community access to health care for Polk County's medically poor and low-income residents in an effort to improve health outcomes and health disparities among residents. The collaboration is a multi-level pilot initiative intended to remove barriers, reduce fragmentation and leverage partnerships.

UWCF will solicit program applications and identify programs appropriate for Behavioral Health and Prevention and Early Intervention pilot programs. UWCF will then manage and administer grants to accepted applicants.

In addition, UWCF's Community Health Navigators will work to enhance local health care navigation and connect residents to the Polk HealthCare Plan, other Indigent Health Care funded safety net organizations, and provide linkage and support to other social services, including, but not limited to, food, housing, and transportation assistance. Navigators will play a key role as connectors, mediators and messengers to vulnerable and marginalized residents who need to be linked to health resources and information so that they can achieve optimal physical and emotional health.

This contract will be effective October 1, 2022 through September 30, 2023 with the option to renew for four one-year renewal terms.

This funding request was presented to and approved by the Citizen's HealthCare Oversight Committee (COC) at the June 17, 2022 meeting.

RECOMMENDATION
Request Board approve the contract with United Way of Central Florida, Inc. to solicit and administer a pilot initiative and provide Navigator services.

FISCAL IMPACT
Funds are budgeted in the Indigent Health Care Funds.
