DESCRIPTION
Amendment Twelve is effective July 1, 2023 and will extend the current contract (FY 2023-2024) and funding for 30 days or until the FY 2024-2025 is approved for Healthy Families Polk. The Amendment will operate without penalties to the funding. The parts of the contract not included in the amendment will remain the same through the end of the contract on July 30,2024. The parts of the contract that conflict with this amendment will be changed to uphold this amendment.
RECOMMENDATION
Request Board approve the Amendment Twelve between the Ounce of Prevention Fund of Florida and Polk County, for the provision of Healthy Families Polk Services.
