DESCRIPTION

CFHC is a Federally Qualified Health Center providing primary and specialty care, dental, and behavioral health services throughout Polk County. CFHC is contracted with the County to provide services to low-income Polk County residents including Polk HealthCare Plan members.

The City of Bartow is an underserved area for health care services with approximately 40% of the population living in households at or below 200% of the federal poverty level. CFHC submitted a request for the funding needed to lease and renovate the 4,400 square foot building to be used as a new community health center in Bartow. The funding request also includes funding for equipment, furniture and supplies to establish the center. CFHC will provide adult primary care and pharmacy services at the new location.

This contract will be effective retroactively from March 1, 2024 to March 31, 2026. This funding request was presented to and approved by the Citizens HealthCare Oversight Committee (COC).

RECOMMENDATION

Approve the contract with CFHC for the building lease, partial funding for renovation costs, and funding for equipment, furniture and supplies for a new community health center in Bartow.

FISCAL IMPACT
