DESCRIPTION

The Florida Department of Health in Polk County provides primary care services in an effort to appropriately link residents to needed medical care to improve overall health and to decrease local emergency room overcrowding by individuals that can best be served in a primary care setting.

Services are provided to qualified low-income Polk County residents at the primary care clinics in Auburndale, Haines City, and Lakeland, as well as virtual telehealth visits.

The current contract is set to term September 30, 2024. This first amendment is to increase the fee schedule, continue funding, and extend the contract term to September 30, 2026.

This funding request was presented to and is recommended for approval by the Citizen’s HealthCare Oversight Committee (COC).
