DESCRIPTION
Central Florida Health Care, Inc. (CFHC) is a Federally Qualified Health Center providing primary and specialty care, dental, and behavioral health services throughout Polk County in Davenport, Dundee, Frostproof, Haines City, Lakeland, Lake Wales, Mulberry and Winter Haven. CFHC is building a new facility to expand services in Frostproof.
CFHC is contracted with the County to provide services to low-income Polk County residents including Polk HealthCare Plan members. CFHC is having a newly constructed facility built for the Frostproof location expanding space and adding more patient rooms.
This contract will be effective January 1, 2024 to September 30, 2024 and will provide funding for the purchase of medical and dental equipment, furniture and supplies for the Frostproof location.
This funding request was presented to and approved by the Citizens HealthCare Oversight Committee (COC).
RECOMMENDATION
Request Board approve the contract with CFHC to provide funding for supplies and equipment needed in the expansion of their Frostproof location as recommended by the COC.
FISCAL IMPACT
Funds are available in the Indigent Health Care Fund.
