DESCRIPTION

Winter Haven Hospital, Inc., a Florida non-for-profit corporation, d/b/a Center for Behavioral Health, operates the Mental Health Hub of Polk County (Hub), located in Winter Haven. The Hub provides site-based services to adults with mental health needs to empower participants for success. Participants can seek services at the Hub for education, support, resources, and skills training. It provides a safe, healthy environment for personal growth, including long-term recovery support, problem resolution, and authentic human connection. Services eligible for reimbursement under this contract are for adult Polk County residents whose income is at or below 200% of the FPL guidelines and who are uninsured or underinsured including Polk HealthCare Plan members.

Services will be provided by:

                     Program Supervisor - master’s degree or higher in a mental health field

                     Case Managers

                     Mental Health Technician

Services include but are not limited to:

                     Education

                     Supportive Counseling

                     Connection to additional Behavioral Health Services

                     Case Management

                     Transportation Connection

                     Skills Training

This contract will be effective retroactively from October 1, 2024 through September 30, 2026. The Center for Behavioral Health will be reimbursed at a rate of $70 per participant per recovery support day.

This request was presented to and recommended for approval by the Citizen’s HealthCare Oversight Committee.

 

RECOMMENDATION

Request approval of the contract with Center for Behavioral Health for site-based services to adults with mental health needs.

 

FISCAL IMPACT

Funds are available in the FY 24/25 Indigent Health Care Funds and will be planned for future years.
