DESCRIPTION

For many years Mid Florida Community Services, Inc., D/B/A You Thrive Florida, a Florida Not for Profit Corporation, headquartered in Brooksville, Florida (hereinafter “MFCS”) has been designated as the Lead Agency for, and has assumed the administration of the State General Revenue grants for the nutritional program for the elderly in Polk County. In January 2023, the County and MFCS entered into Lease No. 345 with MFCS which provides in-kind building space within the Frank B. Smith Human Services Building along with equipment and technical support and also approved Lease No. 346 which provides space within the Wabash Resource Center, all to facilitate its nutritional services program. Both leases were for a term of one year beginning January 1, 2023, and included 3 one-year renewal options.

MFCS previously requested, and the Board approved, its first and second renewal options on both leases which are now due to expire on December 31, 2025. MFCS has requested to exercise its third and final renewal option on both Leases and has executed a Third Amendment to Lease No. 345 Extension Agreement and a Third Amendment to Lease No. 346 Extension Agreement which extends the term of each lease through December 31, 2026, with all other terms, conditions and provisions remaining the same as under the initial Leases. Approving the aforementioned Third Amendments will allow MFCS to continue occupying and utilize the office space and resource center space to provide nutritional services to Polk County’s elderly population.

RECOMMENDATION

Request Board approve the aforementioned Third Amendment to Lease No. 345 and the Third Amendment to Lease No. 346.
