DESCRIPTION
On November 16, 2021, the Board approved the Florida Department of Equal Opportunity (DEO) Federally funded Community Development Block Grant Mitigation Program (CDBG-MIT) subrecipient agreement with the Florida Department of Equal Opportunity for the Polk County Specialty Care/Indigent Healthcare Roof Replacement project. (Federal Funding: $1,060,000.00, County Funding: $0 with total awarded funds of $1,060,000.00)
On July 5, 2022, the Board approved Amendment one (1) which modified Section 15, Citizens' Complaints in the agreement with new language at the request of Florida Department of Equal Opportunity.
On June 21, 2024, the Board approved Amendment two (2) which modified the following:
1. Florida Department of Equal Opportunity (DEO) was renamed Florida Department of Commerce.
2. Section 28, Employee Eligibility Verification language change.
3. Attachment A, Project Description and Deliverables was replaced to include reduction in the scope of work and rename the project Specialty Care/Children' Home Society Roof Replacement Project.
4. Attachment G, Reports, Section 3 language was replaced to meet new closeout procedures.
5. Attachment G, Reports, Section 6 language was replaced to meet new reporting requirements.
Amendment three (3) will modify the following:
1. Section 3, period of Agreement will be extended until June 30, 2026, which allows for a six (6) month extension for project closeout.
2. Section 7, Audit Requirements subsection a, was replaced with new language.
