DESCRIPTION

Since 2017, Community Health Care (CHC) has administered the Helping HANDS (Healthcare Access Navigation Delivery and Support) Program that assists newly released inmates of the Polk County Jail by connecting them with local behavioral health providers and access to appropriate social service programs as the inmates transition from jail into the community. Participants in the Program are qualified Polk County residents. Tri-County Human Services, Inc. partners with CHC to provide services to Program participants.

As Program participants are seeking stable lifestyles, housing, transportation and employment, incidental expenses can occur. In order to remove barriers that impede participant success in the Program, funding through this contract will be used to reimburse for incidental expenses including but not limited to: clothing, hygiene needs, household items, transportation, work-related tools, education, and fees associated with obtaining legal documents.

This contract will be effective October 1, 2025 to September 30, 2026 with the option to automatically renew for two one-year renewal terms.

 

RECOMMENDATION

Approve the contract with Tri-County Human Services, Inc. for the purchase of incidentals for Helping HANDS participants.

 

FISCAL IMPACT

Funds are available in the Polk County Florida Opioid Trust Funds.
