DESCRIPTION

On December 8, 1998, the Board approved the initial contract to partner with The Ounce of Prevention Fund of Florida, Healthy Families Florida to support Healthy Families Polk, which is a home visitation program that promotes positive parenting, self-sufficiency, healthy child development and prevents poor childhood outcomes such as abuse and neglect. Due to the success of the program, Healthy Families has continued to receive funding from The Ounce of Prevention Fund of Florida, Healthy Families Florida and the Board.

 

Amendment thirteen will renew and amend The Ounce of Prevention Fund of Florida contract effective July 1, 2022 through June 30, 2025. The contract number will change to #20-25-05.  Ounce of Prevention Fund of Florida, Healthy Families Florida will provide a one-time non-reoccurring amount of $139,290.75 in addition to the base funding of $1,446,553.00. Polk County, a political subdivision of the State of Florida (Healthy Families Polk), will provide and maintain a minimum of $361,638 (25% cash contribution OFF/HFF Grant Funds). Currently, the Board cash match of $400,000.00 exceeds the minimum in order to contribute to the funding of the salary plan implemented in FY 22-23.

 

RECOMMENDATION

Request board approval of the amendment thirteen between the Ounce of Prevention Fund of Florida and Polk County, for the provision of Healthy Families Polk Services.

 

FISCAL IMPACT

None, funds are budgeted in the Special Revenue Grant Fund.
