Approve the contract with Central Florida Health Care to provide primary care services to eligible low-income county residents who are not Polk HealthCare Plan members. (total amount not to exceed $850,000 annually)

DESCRIPTION
Central Florida Health Care will provide a one-time primary care encounter for Polk County residents seeking medical treatment. Each encounter represents an entry point into the local health care safety net and will generate automatic referrals to the Polk HealthCare Plan to complete the eligibility process.
If the client is approved for the Polk HealthCare Plan, they will have access to a vast network of more than 1,000 providers, approximately 300 access points, with more comprehensive services including, but not limited to, primary care, specialty care, hospital services, urgent care, diagnostics, medications, and more. Primary care clinic locations are in Bartow, Davenport, Dundee, Frostproof, Haines City, Lake Wales, Lakeland, Mulberry, and Winter Haven.
This contract will be effective retroactively from October 1, 2026 to September 30, 2028. Thereafter, the contract will automatically renew for one one-year renewal term, subject to available funding. No additional action is required for renewal unless the contract is otherwise ended in accordance with its terms or the County provides Central Florida Health Care written notice by July 1 that it has elected not to renew for the upcoming term.
Funding for this contract was presented to and is recommended for approval by the Citizen’s HealthCare Oversight Committee.
RECOMMENDATION

FISCAL IMPACT
Funds are available in the Indigent Health Care Funds.
