DESCRIPTION
Hill Orthopedic Center, LLC has been contracted with the County since October 2016 to provide orthopedic services to Polk HealthCare Plan members. Hill Orthopedic Center, LLC is now expanding the services their practice offers. The amendment and restatement to the medical services agreement will allow Hill Orthopedic Center, LLC to provide additional specialty care services to Polk HealthCare Plan members. Additional services include, but are not limited to, physical and occupational therapy, podiatry and wound care.
The amendment and restatement will be effective upon execution and will be ongoing unless terminated pursuant to the agreement.
RECOMMENDATION
Approve the Amendment and Restatement to the Medical Services Agreement with Hill Orthopedic Center, LLC for specialty care services to Polk HealthCare Plan members.
FISCAL IMPACT
Funds are available in the Indigent Health Care Funds.
