DESCRIPTION

On December 8, 1998, the Board approved the initial contract to partner with The Ounce of Prevention Fund of Florida, Healthy Families Florida to support Healthy Families Polk, which is a home visitation program that promotes positive parenting, self-sufficiency, healthy child development, and prevents poor childhood outcomes such as abuse and neglect.

 

Amendment two will renew and amend The Ounce of Prevention Fund of Florida contract effective July 1, 2026 through June 30, 2031. The contract number will change to #25-31-31. 

 

The Ounce of Prevention Fund of Florida through Healthy Families Florida will provide base funding of $1,446,553 and a one-time non-reoccurring amount of $133,945 for a total revenue amount of $1,580,498.
The County (Healthy Families Polk) will provide and maintain the minimum required cash match of $361,639 plus an additional $1,730,899 in order to fund the County’s ongoing salary plan, for a total expense amount of $2,092,538.

 

RECOMMENDATION

Request board approval of Subcontract Amendment two between the Ounce of Prevention Fund of Florida, Healthy Families Florida, and Polk County, a political subdivision of the State of Florida, for the provision of Healthy Families Polk services in Fiscal Year 2026-2027.

 

FISCAL IMPACT

Funds are budgeted in the Special Revenue Grant Fund and General Fund.
