DESCRIPTION

Purpose of Layboard: the Citizens HealthCare Oversight Committee will ensure the integrity of the expenditure of Indigent HealthCare Sales Tax funds and provide a forum for citizen participation in healthcare planning.

Creating Authority: In accordance with Ordinance Nos. 03-89, 04-14, 04-72, 05-23, 08-142, 08-146A, Florida Statute 212.055.

Appointments: The Board shall approve Polk County citizens to serve on the Committee. Members represent Senior Citizen, Work Force (Skilled Labor Representative), Children’s Interest, Business Community, Public Health Director (or designee), Hospital Executive, HealthCare Clinic or Pharmacy, Quality of Life Professional, Primary Care Physician, School Board, Minority Representative, Specialist Physician, and Health Insurance Industry.

Term: Members are appointed for a four-year term. Upon appointment, each member may be requested to serve successive terms beyond the four-year term with Board approval (Resolution Number 23-057)

Reason for Appointment: Expiration of previous term.

Reappoint Daniel Haight, MD as a Member of the Citizens HealthCare Oversight Committee representing Primary Care Physicians, for the four-year term, April 6, 2025 through April 5, 2029.
