DESCRIPTION

Tri-County Human Services, Inc., a Florida not-for-profit corporation (hereinafter “Tri-County”), provides peer support services and assistance to newly released inmates of the Polk County Jail, under a program known as Helping HANDS (Healthcare Access Navigation Delivery and Support) (the “Program”). Tri-County currently leases office space located within the Indigent Health Care Division building located at 2135 Marshall Edwards Drive in Bartow to facilitate the Program. The County and Tri-County previously entered into a lease agreement in 2022 with a current term through September 30, 2025.  Tri-County recently requested to revise the office space by adding Office 270 and removing Office 275 as well as adding additional furniture, equipment and other personalty to the lease.  Tri-County has executed and delivered to the County a First Amendment to Lease No. 362 for the office space revisions and the additional personalty with all other terms and condition of the original lease remaining the same. Approving the aforementioned First Amendment will allow Tri- County to utilize the revised office space and additional equipment for its continued use in support of the Program.

RECOMMENDATION

Request Board approve the aforementioned First Amendment to Lease No. 362.

There is no fiscal impact associated with this agenda.
