DESCRIPTION
Parkview Outreach Community Center, Inc. (Parkview) is one of five free clinics financially supported by the Indigent Health Care sales tax and has contracted with the County since October 1, 2012 to provide services to low-income Polk County residents. Free Clinics act as a diversion program that helps to decrease local emergency room overcrowding by providing a medical home for individuals that can best be served in a primary care setting. Parkview is located in Haines City and services are provided by staff and more than 21 volunteers including physicians, nurse practitioners, medical providers, administrative and ancillary staff. Medical services include, but are not limited to, primary care, specialty care, ancillary and diagnostic services, and pharmacy services.
This amendment and restatement updates the contract’s eligibility provisions by removing the current limitation of 200% of the Federal Poverty Level and providing that eligibility will instead be determined in accordance with applicable Florida Statutes and the County Ordinance, as amended.
The amendment also extends the contract term effective October 1, 2026. Thereafter, the contract will automatically renew for successive one-year terms beginning October 1 of each year, subject to available funding. No additional action is required for renewal unless the contract is otherwise ended in accordance with its terms or the County provides Parkview written notice by July 1 that it has elected not to renew for the upcoming term.
This funding request was presented to and is recommended for approval by the Citizen’s HealthCare Oversight Committee.
