DESCRIPTION
The amendment to the behavioral health services agreement will allow Silver Lining Counseling Kerrick Foundation, Inc. to continue providing behavioral health care services to qualified county residents enrolled in the Polk HealthCare Plan.
The agreement was effective June 1, 2024 through September 30, 2025. This first amendment effective October 1, 2025 extends the term of the agreement through September 30, 2028, adjusts the fee schedule, and allows for more billable services.
RECOMMENDATION
Approve the first amendment to the Polk HealthCare Plan Behavioral Health Services Agreement with Silver Lining Counseling Kerrick Foundation, Inc. for behavioral health care services.
FISCAL IMPACT
Funds are available in the Indigent Health Care Funds.
