DESCRIPTION

The Lake Wales Free Clinic is one of five free clinics financially supported by the Indigent Health Care sales tax and has contracted with the County since October 2012 to provide services to low-income Polk County residents. Free Clinics act as a diversion program that helps to decrease local emergency room overcrowding by providing a medical home for individuals that can best be served in a primary care setting. Medical services include, but are not limited to, primary care, specialty care, and ancillary and diagnostic services.

This amendment and restatement to the contract extends the term of the contract and will be effective retroactive to October 1, 2025. The contract will automatically renew for one-year renewal terms.

This funding request was presented to and is recommended for approval by the Citizen’s HealthCare Oversight Committee.

 

RECOMMENDATION

Approve the amendment and restatement to the contract with Lake Wales Care Center, Inc. for the provision of primary and specialty care services at the Lake Wales Free Clinic.

 

FISCAL IMPACT

Funds are available in the Indigent Health Care Fund.
