DESCRIPTION
The medical services agreement will allow for the provision of vascular medicine and surgery services to qualified county residents enrolled in the Polk HealthCare Plan. The agreement will be effective December 1, 2024.
RECOMMENDATION
Request approval of the medical services agreement with Vein & Vascular Experts, PLLC.
FISCAL IMPACT
Funds are available in the FY 24/25 Indigent Health Care Funds and will be planned in subsequent years.
