DESCRIPTION
This is a name change from “Affordable Transport, Inc.” naming on the COPCN to “MedFleet LLC, dba ATI Ambulance,” after an asset purchase of Affordable Transport, Inc. This transaction occurred on March 1, 2024, and MedFleet, LLC. is now the parent company and will be “Doing Business As” ATI Ambulance.
MedFleet LLC dba ATI Ambulance currently has a COPCN to operate in Polk County. Per Polk County 12-029, the type of service provided is Type G, which is specifically used for Advanced Life Support (ALS) medically necessary inter-facility transport ambulance service, and Type H, which is specifically used for Basic Life Support (BLS) medically necessary inter-facility transport ambulance service.
