DESCRIPTION

In response to a Request for Applications through Indigent Health Care, CFHC submitted a request for partial funding for the construction of a new community health center in Lakeland. CFHC operates three health care facilities in Lakeland: Lakeland Primary Care (N Missouri Ave), Lakeland Pediatrics (S Florida Ave), and Lakeland Hills Dental (Lakeland Hills Blvd). The new facility will consolidate the three clinics into one location adjacent to the current N Missouri Avenue location.

CFHC is a Federally Qualified Health Center providing primary and specialty care, dental, and behavioral health services throughout Polk County. CFHC is contracted with the County to provide services to low-income Polk County residents including Polk HealthCare Plan members. Currently, CFHC’s three Lakeland facilities have a combined square footage of 18,060. The new build will be a 33,600 square foot, four-story, state-of-the-art health center. Services provided will include adult and pediatric primary care and dental services, gynecology, behavioral health, labs, x-ray, and a drive through pharmacy. Additional telehealth services will be provided for endocrinology and behavioral health.

This contract will be effective January 1, 2024 to December 31, 2026. This funding request was presented to and approved by the Citizens HealthCare Oversight Committee (COC).

 

RECOMMENDATION

Approve the contract with CFHC for the partial funding for the construction of a new community health center in Lakeland.

 

FISCAL IMPACT

Funds are available in the Indigent Health Care Fund.
