DESCRIPTION
In response to a Request for Applications through Indigent Health Care, CFHC submitted a request for partial funding for the construction of a new community health center in Dundee.
CFHC is a Federally Qualified Health Center providing primary and specialty care, dental, and behavioral health services throughout Polk County. CFHC is contracted with the County to provide services to low-income Polk County residents including Polk HealthCare Plan members. Currently, CFHC’s Dundee facility has 8 exam rooms and is a 5,208 square foot building. The new build will be on the same property and will be a 9,391 square foot, two-story, state-of-the-art health center with 13 medical exam rooms and 5 dental operatories. The larger facility will allow for an expansion of adult primary care and dental services and the addition of pediatric primary care and dental services, and a drive through pharmacy. Additional telehealth services will be provided for endocrinology and behavioral health.
This contract will be effective January 1, 2024 to December 31, 2026. This funding request was presented to and approved by the Citizens HealthCare Oversight Committee (COC).
RECOMMENDATION
Approve the contract with CFHC for the partial funding for the construction of a new community health center in Dundee.
FISCAL IMPACT
Funds are available in the Indigent Health Care Fund.
