DESCRIPTION
Amendment #0008 is effective April 1, 2023. The changes include a reduction of HFF Grant funds due to staff vacancies, funding movement among approved categories, and revised budget with associated funding changes. The parts of the contract not included in this amendment will remain the same through the end of the fiscal year ending June 30, 2023. The parts of the contract that conflict with this amendment will be changed to uphold this amendment.
RECOMMENDATION
Request board approval of the Amendment #0008 between the Ounce of Prevention Fund of Florida and Polk County Board of County Commissioners, for the provision of Healthy Families Polk Services.
FISCAL IMPACT
Funds are available in the Special Revenue Grant Fund.
