DESCRIPTION
Amendment #9 is effective July 1, 2022 and will extend the current contract (FY 2022-2023) and funding for 30 days or until the FY 2023-2024 contract is approved for Healthy Families Polk. The Amendment will operate without any penalties to the budget. The parts of the contract not included in this amendment will remain the same through the end of the contract July 30, 2023. The parts of the contract that conflict with this amendment will be changed to uphold this amendment.
RECOMMENDATION
Request board approval of the Amendment Nine between the Ounce of Prevention Fund of Florida and Polk County, for the provision of Healthy Families Polk Services.
