DESCRIPTION
In response to a Request for Proposal through Indigent Health Care, LRH submitted a proposal to provide specialty care services and navigation services.
LRH has a residency program operating out of LRH's Parkview location in Lakeland adjacent from the hospital and LRH's Morrell Internal Medicine primary care clinic. Medical residents participating in the program will do a rotation with each specialty provider at Parkview. LRH will provide specialty care services to qualified Polk County residents at or below 200% of the Federal Poverty Level and who are uninsured or not otherwise PHP members. Specialty care services include, but are not limited to, cardiology, gastroenterology, general surgery, nephrology, neurology, orthopedic surgery, pulmonology and pharmacy.
In addition to specialty care services, LRH will have six Navigators who will screen, connect and provide direct enrollment assistance to qualified Polk County residents to PHP and community partner resources within Polk County. Navigators will be located at LRH's Morrell Internal Medicine, Family Medicine Residency Clinic at Kathleen, Lakeland Regional Medical Center hospital and the hospital Emergency Department (ED). Providing residents with navigator services will connect them with a medical home, specialty care services, and reduce overuse of the ED.
This contract will be effective April 1, 2024 with the option to automatically renew to September 30, 2026.
This funding request was presented to and approved by the Citizens HealthCare Oversight Committee.

RECOMMENDATION
Approve the contract with LRH for specialty care services to non-PHP members and navigation services.
